Benefits

Section A.

During the term of this Agreement, and consistent with the terms of each program or plan, employees with a one-quarter or greater employment fraction in a term are eligible to participate in the University's Group Health Care programs and Group Dental Option I Plan, and Basic Group Life Insurance plans. University contributions toward the Group Health and Group Dental premiums shall be in the same amount as that provided to the University instructional staff for the coverage selected. The University will ensure that the same-sex domestic partners of employees are eligible for group health and group dental coverage in accordance with Article IV Section A of this contract. The University's contribution for Basic Group Life Insurance coverage will be the same as for instructional staff. In the event of any changes in the coverage from any of the programs or plans, the GEO Steering Committee will be notified sixty (60) days prior to the effective date of the change.

Application for Group Health Care programs and Basic Group Life Insurance coverage must be made at the Benefits Office within sixty (60) days of the effective date of employment. Applicants must provide the Benefits Office with proof of eligibility in order to be considered for coverage. For the purpose of this Article, proof of eligibility is considered to be a departmentally approved: Request for Appointment, Form 36400; Request for Appointment Change, Form 36701; a Notice of Terms and Conditions of your current appointments, or a Fraction Calculation form, Form 30010 or Form 30011.
Not later than the 30th day after employment begins, the University will notify an employee of the provisions of this Article including a reference to the time period requirement for application. In cases where an employee misses the application deadline due to a lack of such notification, the University will make every effort to assist that employee in enrolling for benefits.

An employee who is participating in the Health, Dental, or Life Insurance plans during Term II, even if employed for IIIA and/or IIIB, and has been re-employed for the following Term I with a one-quarter or greater employment fraction may continue the Group Health Care programs, Group Dental Option I Plan, or Basic Group Life Insurance coverage during the months of May, June, July, August, and September between Term II and Term I by ensuring that confirmation of the Term I employment is delivered to the Benefits Office by April 10.

An employee who is not employed for Term II but is employed for Term IIIA and re-employed for the following Term I and has a one-quarter time or greater employment fraction for both terms may continue the Group Health Care programs, Group Dental Option I Plan, or Basic Group Life Insurance coverage during the months of July, August, and September between Term IIIA and Term I by making arrangements through the Benefits Office by June 30, provided the initial employment and re-employment occur no later than the first day of Term IIIA.

Effective September 1, 1999, Group Dental Option I Plan coverage is effective with the beginning of a term of employment with a one-quarter or greater employment fraction following a waiting period of two (2) cumulative terms of employment with a one-quarter or greater employment fraction within a two (2) year period. Following a lapse in employment of more than eight (8) months reinstatement of dental coverage will require an employee to notify the Office of Academic Human Resources in writing of the return within sixty (60) days of the beginning of the term. Dental coverage following such a lapse in employment will begin on the later of these dates: the date first eligible for dental coverage (as stated above), or, the date a completed enrollment form is received by the Benefits Office. However, if a lapse of employment of five (5) years between appointments occurs, a new waiting period will be established before dental coverage is reinstated. During any term of employment at a one-quarter or greater appointment that falls within a waiting period, an employee may, within sixty (60) days of beginning work, elect to participate in the University Group Dental Option I Plan at the same buy-in rate as other instructional staff.

Section B.

Effective January 1, 2003, any GSI/GSSA who is appointed with an employment fraction of below .25 throughout not less than a whole term, may purchase Grad Care medical insurance plan (single, two party or family) at their own expense.

Effective January 1, 2003, any GSI/GSSA who is appointed with an employment fraction below .25 throughout not less than a whole term, may, at their own expense, purchase Group Dental Option 1 plan coverage following a waiting period of two (2) cumulative terms of employment within a two (2) year period.

In either case, coverage begins in the first month in which:
1) Both an appointment notification is issued and the Benefits Office receives a completed application for the benefit from the GSI/GSSA And

2) The GSI/GSSA makes appropriate payment arrangements with the Benefits Office

Section C.

Effective September 1, 2002, and for the duration of this Agreement, the University agrees to the following:

Any GSI/GSSA who meets the eligibility requirements for the Childcare Subsidy Program will receive the appropriate subsidy award.

The maximum Subsidy award per term for each GSI/GSSA who meets the eligibility requirements specified above will be:

First child: $1,700
Second Child: $850
Third child: $850
To fiscally assure subsidy awards are available, $50,000 will be added to the Childcare Subsidy Program each year of the agreement.
